Guidance and tool for people with learning disabilities

People with learning disabilities helped us to make this guidance and tool. 

We did a workshop with people with learning disabilities who told us what they would like to know to prepare for a Care Act assessment. They also helped us to make the guide and tool into Easy Read.

The guide helps you to know what a Care Act assessment is. This includes what you will be asked, what your rights are, and what you can ask for to help you.

The Menu for Support tool helps you to think about what support needs you have so that you can talk about these in your Care Act assessment.
